CHRONIC BRONCHITIS

 

DEFINITION:

A chronic inflammatory disease of the bronchi caused by chronic bronchial irritation characterized by cough and chest pain.

EPIDEMIOLOGY:

  • incidence: ?
  • age of onset:
    • childhood -> adolescence (chronic cough)
  • risk factors:
    • allergies
    • agents causing chronic bronchial irritation:
      • air pollution (car exhausts, industrial fumes)
      • smoking (tobacco, marijuana)
      • wood-burning stoves

PATHOGENESIS:

1. Background

  • existence of this entity is questionable
  • usually associated with an underlying cause:
    • immunodeficiencies
    • decreased mucociliary clearance
      • CF, Ciliary Dyskinesia Syndrome
    • chronic respiratory diseases
      • asthma, bronchiectasis, environmental disease, upper airway infection
  • difference between this entity and asthma is that the former is usually associated with expectorate and chest soreness

CLINICAL FEATURES:

1. Respiratory Manifestations,

1. Chronic Cough

  • frequently recurring
  • usually with expectorate with "casts"
  • worse at night

2. Others

  • chest soreness
  • chronic wheezing

INVESTIGATIONS:

1. Pathology

1. Casts

  • large solid hypereosinophilic mucoid masses
  • contain metaplastic bronchial epithelium
  • contain inflammatory cells & noncellular material

MANAGEMENT:

1. Supportive

  • treat underlying causes, i.e., asthma
  • treat allergies
  • avoid irritants, i.e., cigarette smoke
