BENIGN PAROXYSMAL VERTIGO
DEFINITION:
A type of migraine characterized by paroxysms of vertigo and
nystagmus.
EPIDEMIOLOGY:
- incidence: ?
- age of onset:
- toddler (rare over the age of 3 years)
- risk factors:
- familial
- family history of migraine in almost all cases
CLINICAL FEATURES:
- sudden onset and associated with ataxia
- usually child unable to sit, stand, or walk
- associated symptoms:
- frightened and pale
- horizontal nystagmus
- motion sickness
- nausea and vomiting
- normal level of consciousness
- rotational sensation
- episodes vary in:
- duration - seconds to minutes
- frequency - daily to monthly and irregular
- intensity - mild to severe
- susceptible to motion sickness
2. Migraines
- episodes of vertigo are later replaced by typical features
of migraine:
- headache
- nausea and vomiting
INVESTIGATIONS:
1. Ice Water Caloric Test
- detects abnormal vestibular function
- all other investigations are normal
MANAGEMENT:
1. Supportive
- Gravol po, IM, IV, or per rectum
- standard migraine therapy for migraines
