BENIGN PAROXYSMAL VERTIGO

 

DEFINITION:

A type of migraine characterized by paroxysms of vertigo and nystagmus.

EPIDEMIOLOGY:

  • incidence: ?
  • age of onset:
    • toddler (rare over the age of 3 years)
  • risk factors:
    • familial
      • family history of migraine in almost all cases

CLINICAL FEATURES:

1. Neurological Manifestations

1. Vertigo

  • sudden onset and associated with ataxia
  • usually child unable to sit, stand, or walk
  • associated symptoms:
    • frightened and pale
    • horizontal nystagmus
    • motion sickness
    • nausea and vomiting
    • normal level of consciousness
    • rotational sensation
  • episodes vary in:
    • duration - seconds to minutes
    • frequency - daily to monthly and irregular
    • intensity - mild to severe
  • susceptible to motion sickness

2. Migraines

  • episodes of vertigo are later replaced by typical features of migraine:
    • headache
    • nausea and vomiting

INVESTIGATIONS:

1. Ice Water Caloric Test

  • detects abnormal vestibular function
  • all other investigations are normal

MANAGEMENT:

1. Supportive

  • Gravol po, IM, IV, or per rectum
  • standard migraine therapy for migraines
