ACUTE UVULITIS

 

DEFINITION:

An acute bacterial infection of the uvula characterized by fever, pain with swallowing, and drooling.

EPIDEMIOLOGY:

  • incidence: rare
  • age of onset:
    • childhood
  • risk factors: ?

PATHOGENESIS:

1. Pathogens

1. Group A Streptococcus (GAS)

  • usually associated with tonsillitis in children > 5 yrs

2. H. influenzae type b (Hib)

  • usually associated with epiglottitis in children < 5 yrs

CLINICAL FEATURES:

1. Respiratory Manifestations

  • fever
  • pain with swallowing
  • drooling

INVESTIGATIONS:

1. Microbiology

  • cultures from uvula and pharynx
  • blood culture

MANAGEMENT:

1. Antibiotics

  • GAS - Penicillin V 25 mg/kg/d po tid x 10 days
  • Hib - Cefuroxime 75 mg/kg/d IV q8h

 

2. Supportive

  • if airway obstruction
